Community Based Services

Respite
We understand that caregiving is both rewarding and demanding. Our Respite services offer trusted relief, allowing caregivers to rest or live their lives while their loved ones receive attentive, high quality, respectful support.

Whether it's helping with daily tasks, companionship, or supervision, our trained staff deliver care with compassion and consistency. Quality means reliability, respect and safety. Empowerment means giving families and caregivers the confidence to take care of themselves, knowing their loved ones are in good hands.

We offer respite services through Kentucky Medicaid Waiver Programs or private payment.

Personal Assistance
We understand that each individual's needs are different and based on their unique capabilities and skills. This is the basis for our personal care and personal assistance services. These services are typically offered in the your home and tailored to your needs. Personal Assistance includes but is not limited to activities of daily living such as bathing, dressing, hygiene, meal preparation, attending medical provider appointments, and a host of other services. These support services allow you to remain in your home with right support to allow you to live life with as much independence as possible, as well as with dignity and respect.

We offer these services through Kentucky Medicaid Waiver Programs or private payment.

Community Living Supports (CLS)
Independence looks different for everyone and Milestones honors that. Our Community Living Supports helps individuals navigate daily life at home and in their communities while helping gain the skills necessary to increase their independence. Our training and support services offer skill development in independent living such as cooking and meal preparation, home cleaning and maintenance, budgeting and appropriate socialization. We build customized plans that reflect each person's skills, goals and preferences.

With our rigorous quality standards, families can trust that support is both professional and personal. Empowerment and skill development is at the heart of CLS, helping individuals make choices, build skills and routines, understand how to navigate their community, and live life on their terms.

Community Access
We believe that everyone deserves a vibrant, connected life. Our Community Access services open doors to art, culture, recreation, and friendship. Whether it's joining a class at a community organization or fitness center, attending a local event, or exploring a new hobby, we make sure individuals feel included and inspired. Quality means safe, fulfilling and well-organized experiences. Empowerment means encouraging self-expression and helping individuals discover their place in the community through garnering respect and developing friendships.

Milestones is proud to deliver these services that align with Medicaid Home and Community Based waiver programs, including SCL and Michelle P. Waiver. Our team understands the nuances of eligibility, documentation, and person-centered planning required for quality of care and compliance.
